dude, there is a few more things going on then this right now. 
They will be mailed a questionnaire I got mine in 2010 and they did not have to notarize. unsure if that changed. I doubt it though.
put target, hunting and all lawful purposes. do not put for self defense. I would not specify for home as thats the default in Rockland anyway. it will be a sportsman/residence restricted license.
