In November, I will be 21 so I will be applying in New York City. I am a Ridgewood, Queens resident!

I have searched, and I couldn't find the right answer to one of the questions that I have for the NYC Premise permit. In the past I have suffered with concentration, and depression. Never arrested. I was hospitalized 8 years ago (Not court ordered) for an evaluation.

At the time I was dealing with ACS, being taken away from my mother was hard and confusing at the time. So i said that I wanted to kill myself, because I was very upset (I was 12 years old) the social worker overreacted and called EMT.

When applying would it be best if I contact a doctor to check over my medical history? I am no longer taking medication because I no longer need it, though I have taken medication. I have been med free for 5 years and I am doing very well. Should I be worried about the application?

Also, is it best if I check off yes since I have been hospitalized and medicated? As far as I see it, the doctors I have had has diagnosed me as bipolar I believe?, and some said I wasn't diagnosed with it at all. I am confused and I have no idea what is on record. Its like I was seeing 3 different types of doctors at once then they all just disappeared?? lol

I am older now to look into this, and any of your help will be greatly appreciated.
